Transcript Requests

The Ontario Student Transcript (OST) is the official record of a student's secondary school academic achievement in the province of Ontario. Student records are confidential, and transcripts are issued only on the written request of the student or parent/guardian (when student is under 18 years of age) and are subject to the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA).

If the last secondary school you attended was within the Dufferin-Peel Catholic District School Board (Ji8��) and you require your transcript, please use the process below that best applies to you to request your Ontario Student Transcript.

1. Applying to an Ontario College through the Ontario College Application Service (OCAS)

If you are a former student of the Ji8�� and are applying to an Ontario college through the Ontario Application Service (OCAS), request your OST using the online electronic transcript request service available through OCAS.

To use this service you must:

Be a past student of the Ji8��
Apply to a minimum of one Ontario college
Pay the college application and transcript fees

To order your transcript online, go to OCAS.ca

NOTE: If you are a current student attending a secondary school within the Ji8��, and are applying to Ontario colleges through OCAS, you do not need to request your transcript. Your transcript information will be forwarded to OCAS on your behalf. College applicants who left day school in June 2023, summer school marks and credits will be electronically transmitted and be available to OCAS and from there will be distributed to your selected colleges.

2. To Request your Transcript for all Other Purposes

TRANSCRIPT REQUESTS FOR THE 2022-2023 and 2023-2024 SCHOOL YEARS:
Requests for transcripts for 2022-2023 and 2023-2024 are processed by the Guidance Department of the student's current or last secondary school attended. Please complete the Transcript Request Form and email the form with a legible copy of your identification directly to the last secondary school you attended within the board. Visit the board website to obtain the email addresses of secondary schools within the Ji8��. Transcripts are available for pick-up at the school or can be emailed or mailed directly to the requester.

ALL OTHER TRANSCRIPT REQUESTS:
Requests for transcripts prior to the 2022-2023 school year are processed by Ji8��'s Admissions Department. Please complete the Transcript Request Form and email it to transcripts@dpcdsb.org and pay the applicable transcript request fees* using SchoolCashOnline. Transcripts will be forwarded directly to the email address of the college/university or other requested post-secondary institution to which you are applying. You will be notified once the transcript has been sent and forwarded any replies received. If you are requesting your transcript to be mailed to your home address, please include your full mailing address on the Transcript Request Form.

*Transcript request fees apply to transcripts prior to the 2023-24 school year. Fees can be paid online through SchoolCashOnline. Transcript Fees: First copy $20 CDN, additional copies $5 CDN each.

Confirmation of Graduation – Diploma Letter Requests

Diploma letters for students who graduated prior to the 2023-2024 school year are processed by Ji8��'s Admissions Department. Please complete the Diploma Letter Request Form and email it to transcripts@dpcdsb.org and pay the applicable fees* using SchoolCashOnline. Diploma letters will be mailed to your home address. Please include your full mailing address on the Diploma Letter Request Form.

*Diploma request fees ($25 CDN) apply to letters prior to the 2023-24 school year. Fees ($25 CDN) can be paid online through SchoolCashOnline.

Secondary Student Activities Fees for the 2024-25 School Year

The Student Activities Fee of $45.00 is an annual fee that is used to offset costs for school initiatives, materials and student activities. The support of our Parents/Guardians through the payment of the Student Activities Fee makes it possible for our school to continue to provide many of the valuable initiatives, materials and activities that enhance the school experience for our students. Our school and the Ji8�� are committed to ensuring every student has equal opportunity to benefit from the school environment, regardless of financial hardships or barriers. The Student Activities Fee helps to offset some of the costs of the following items:

Support for clubs, teams and activities (entry fees, equipment, transportation to events, trophies/awards, club/team uniforms).
Guest speakers and assemblies (Anti-bullying initiatives, mental health awareness).
Student Council (supports for speakers, special event days).
Chaplaincy initiatives, faith formation and supports for families in need.
Support for student enrichment opportunities and leadership Initiatives.
School beautification.

Indigenous Self-Identification

Voluntary Indigenous Self-Identification
Ji8�� Board is providing an opportunity for students to self-identify their First Nation, Métis and/or Inuit ancestry in a voluntary and confidential manner. This program will allow the Board to provide expanded supports to First Nation, Inuit and Métis students.

Why self-identify?
Self-identification will provide access to culture and language programs, funding opportunities through potential scholarships and bursaries, and monthly programming for students and families. Relevant information will be sent directly to you about upcoming cultural events and opportunities which help to build community and a sense of Indigenous pride in Dufferin-Peel. Indigenous secondary students can decide to participate in the Ji8�� Indigenous Student Council as well as provide valuable input regarding Indigenous Education throughout the board.

Who can self-identify?
If you are First Nation (Status Indian or Non-Status Indian), Métis and/or Inuit you can self-identify as Indigenous. You do not need to produce any documentation or proof of ancestry. Self-identification is voluntary and confidential.

How can I self-identify?
There are several ways to indicate your identity if you should choose to identify.

1. New students: the Ji8�� online registration form has a section where you can indicate if you identify as First Nation, Métis and/or Inuit.

2. Returning students: can register by updating their student verification form that are sent home each year to give parents the opportunity to update information. There is a section on this form where you can self-identify.

3. Complete the online form.

Who will have access to my information?
This data will be recorded in the Board's Student Management System in a personal and confidential manner. The Indigenous Education Department of the Board will have access to contact information in order to connect students and families with cultural events and other relevant opportunities. This information will not be shared and/or used outside of the Indigenous Education department without the express, free, prior and informed consent of the student and/or family. School principals will also have access to this information, and will remain confidential unless families choose to disclose this with other staff such as a student's classroom teacher.

Catholicism's Core Understanding of the Human Condition

Distinctive expectations for graduates of Catholic schools are determined and shaped by the vision and destiny of the human person emerging from our faith tradition. This Christian anthropology or world view, reveals the dignity and value of the person. Our tradition tells us God creatively and lovingly calls each of us into the wonder of life, sustaining us by the power of the Holy Spirit, throughout the human journey, into life eternal. We acknowledge that the journey includes moments of brokenness and sin.

We recognize in the person of Jesus, the risen Christ, the human face of God sharing our life in order to heal us of our brokenness and liberate us from sin. This Christian vision of the human journey is best understood within the context of relationship. It is accomplished in community, in solidarity with brothers and sisters in the Church and beyond.

Catholic education views human life as an integration of body, mind, and spirit. Rooted in this vision, Catholic education fosters the search for knowledge as a lifelong spiritual and academic quest. The expectations of Catholic graduates, therefore, are described not only in terms of knowledge and skills, but in terms of values, attitudes and actions.

Vision of the Learner

The Graduate is expected to be:

A discerning believer formed in the Catholic Faith community who celebrates the signs and sacred mystery of God's presence through word, sacrament, prayer, forgiveness, reflection and moral living.
An effective communicator who speaks, writes and listens honestly and sensitively, responding critically in light of gospel values.

Develops attitudes and values founded on Catholic social teaching and acts to promote social responsibility, human solidarity and the common good.

Seeks intimacy with God and celebrates communion with God, others and creation through prayer and worship.

2. AN EFFECTIVE COMMUNICATOR WHO:

Presents information and ideas clearly and honestly and with sensitivity to others.
Writes and speaks fluently one or both of Canada's official languages.
Uses and integrates the Catholic faith tradition, in the critical analysis of the arts, media, technology and information systems to enhance the quality of life.

3. A REFLECTIVE AND CREATIVE THINKER WHO:

Recognizes there is more grace in our world than sin and that hope is essential in facing all challenges.
Creates, adapts, evaluates new ideas in light of the common good.
Thinks reflectively and creatively to evaluate situations and solve problems.
Makes decisions in light of gospel values with an inform-ed moral conscience.
Adopts a holistic approach to life by integrating learning from various subject areas and experience.
